HI-TEE Jr Little League Golf Program and all of its members, officers, agents, directors, escorts, successors, assignees, are to be free from any and all liability claims of any kind or other forms of legal action, including costs arising from any accidents which results in or arises out of any loss of property or person, including death, which may be sustained by the Junior or parent/guardian while engaged in or traveling to or from HI-TEE Jr Little League Golf sponsored events. The undersigned parent/guardian give the HI-TEE Jr Little League, its program assistants, club member, program managers, and assistants, or any physician selected by the same, the rights to authorize emergency medical treatment for the Junior named above herein while said Junior is participating in any of the HI-TEE Jr Little League Golf Program or member club sponsored events. I/We the undersigned parent/guardian do by submitting the application, acknowledge that I/WE have read, understand and accept the terms and provision contained herein. I/We understand that the Juniors membership in the program may be suspended or canceled for falsification of information on the application or willful disregard of the rules instituted by the HI-TEE Jr Little League Golf Program. I/We agree to allow the use of any photographs and/or images of my child participating in any sponsored events for promotional and/or marketing materials or purposes.
